Please read below and decide which forms you need to fill out and return to us. Please note that you must have Adobe Acrobat Reader in order to view these files.
If you are a NEW Member:
Welcome! We’re glad you’re joining us! Since you’re a new member, you will need to fill out both the 5 Page Member Form and the Regional Questionnaire. You must also go to www.ihsainc.com and become a member. DO NOT make a payment online, you must print out the page and bring cash or a check to your team captain. Feel free to take a look at What Is UFEC? to find out more about us. If you have questions regarding the forms, please leave them blank–we will help you fill them out!
If you are a RETURNING Member:
We’re glad to have you back! Although you’ve already filled out the 5 Page Member Form, we still need you to fill out the Regional Questionnaire. You must also renew your IHSA membership at www.ihsainc.com. Click on the Membership tab at the top left. Renew the individual (rider) membership. Please do not pay online, bring cash or a check to your team captain. Return your two forms and cash or a check for your IHSA membership to your captain as soon as possible.
